What if my sexual wellness sample cannot be processed?

TO ENSURE YOUR SAMPLE IS PROCESSED BY THE LAB, YOU MUST:

  1. Complete and apply the sample label to the tube with your registered name, date of birth, and date of collection.
  2. Ensure that the tube is completely closed after the urine is added to the collection tube.
  3. Add sample label to the urine collection tube
  4. Adhere to the instructions provided in the package insert.
  5. Ensure your sample is successfully registered at app.simplehealthkit.com before shipping out the sample.

Your sample may not be processed, and you may not receive a test result if you do not follow the instructions. Please reach out to your testing administrator or contact help@simplehealthkit.com for more information.
