How do I register my Sexual Wellness Test to my portal?
To begin registering, you will need
- Your test kit and your insurance information
- (If you are uninsured, contact your admin or Simple HealthKit help@simplehealthkit.com)
There are five steps for registering your test:
- Entering kit detail
- Providing patient information
- Answer screening questions
- Sample collection information
- Review the information and submit it!

Providing patient information
If you have not added a patient yet, to add a patient, go through the following steps in the pop-up ADD A PATIENT form
- Sign the consent form and disclaimer
- Provide medical data
- Provide insurance data (If you are uninsured, contact your admin or Simple HealthKit support@simplehealthkit.com)
