How to create an account as a new patient?
Before registering a kit, register yourself as a patient. You will need your insurance information (If you are uninsured, contact your admin or Simple HealthKit help@simplehealthkit.com)
1. Go to app.simplehealthkit.com and create an account
2. Enter your information and insurance details
3. If applicable, enter your Site Code if you are testing as a part of an organization
